Online Credit Application

Thank you for your interest in Mid-West Fertilizer. To do business with us, an application form must be completed and returned to our office.

An email address is required to complete a credit application online and sign the document electronically. If no email is available, a paper application can be downloaded, completed and mailed to our office.

Oklahoma residents must send a copy of sales tax exemption permit to

mwfcredit@mwfert.com.

Without proof of exemption on file, all Oklahoma sales will be considered taxable.

Strengthening community connections

Mid-West Fertilizer is committed to helping our rural communities prosper and prepare for tomorrow. Developing leaders within our communities, alleviating hunger, and enhancing ag education and community safety are the foundational areas we proudly support.

We routinely make donations of money and time to programs that include 4-H Leadership Development, community food banks, FFA, Ag in the Classroom, community festivals and more.

Organizations requesting donations from Mid-West Fertilizer must complete an application. Please allow 5-7 business days for a response from Mid-West Fertilizer. When the form has been submitted correctly, a confirmation will be sent using the email provided in the form. To make a request, contact your local Mid-West Fertilizer location.

Fostering the next generation of cooperative leaders

Mid-West Fertilizer offers annual scholarships to deserving high school seniors and college students. To be eligible for a Mid-West Fertilizer scholarship, students must be planning to attend or return to an accredited university or college including a community, vocational or technical school.
